Chapter 8

Chapter 8

Clear fluid suddenly rushed out.

The atmosphere in the operating room changed.

For a moment, everyone seemed to stop breathing.

Im Gain slid her hands inside and pulled the baby's shoulders free.

"No forceps. I'm delivering directly. Baby coming out!"

A limp baby was pulled from the mother's abdomen.

The baby's lips were blue.

Exactly as expected.

If the mother had no spontaneous circulation for four to five minutes after cardiac arrest, the fetus would also lose oxygen during that period.

The baby's oxygen came through the placenta.

I only need to focus on the mother.

The limp baby.

Blue lips.

A tiny body born without a heartbeat.

The neonatal team will handle the baby.

Even as she told herself that, Gain's heart hurt.

The baby's condition was terrible.

The likely outcome was obvious.

This child needed a miracle too.

But Gain's spring of divine power was exactly as she'd discovered in the market.

Dry.

There was nothing left to give.

She missed her divine power.

I'm sorry.

What I can give you right now is...

Then it happened.

At the same time, on the seventh-floor OB-GYN ward of Hanguk University Hospital.

Grandma Malseon, who sold mugwort at the market, was lying in bed listening to the guardian beside her chatter nonstop like a radio.

"I heard a full-term pregnant woman came into the ER with her heart stopped. Apparently some young, pretty lady doctor had been doing chest compressions since the market and rode in with her."

"A young lady doctor?"

Grandma Malseon suddenly sat upright.

A young female doctor from the market?

Could it be the woman who had saved her?

She remembered the warm hand that had held hers on the day her seasoned mugwort spilled onto the floor.

That's my doctor.

It had to be.

That doctor was exactly the kind of person who would do something like that.

The grandmother clasped both hands together.

She had practiced Buddhism for forty years.

All she'd ever prayed for was that her dead husband would reach paradise and that her children would remain safe.

For the first time, she prayed for a complete stranger.

Buddha, God, anyone will do. Please hold my doctor's hand. Those beautiful hands save people. She even saved an old woman like me...

Her gratitude became a prayer.

In the operating room.

Gain's fingertips suddenly became hot.

?!

From the dry bottom of that little spring...

Water appeared.

One drop.

Then another.

Warmth rose from somewhere she couldn't identify.

It gathered near her heart, traveled down her arm, and reached her fingertips.

No halo.

No angels.

No voice of the Goddess.

It simply...

Came.

What is this?

Why now...?

Was it because she had prayed?

No.

She had been praying constantly before this.

There was no time to investigate.

A blessing of unknown origin was still a blessing.

Gain took that tiny amount of power and directed it into the baby.

She calculated everything.

The path of blood circulation.

The direction of the nerves.

The thalamus.

The hippocampus.

She wasted nothing.

Live.

The pediatric nurse immediately took the baby and began chest compressions.

The pediatrician secured the airway.

If the baby is delivered within five minutes of the mother's cardiac arrest, recovery after CPR is possible.

But after five to ten minutes, the baby can be born in cardiac arrest...

And after fifteen minutes, like this baby, recovery can become impossible, or severe brain damage becomes highly likely.

Now Gain needed to return her attention to the mother.

She forced her slightly trembling hands to become steady again and checked the patient.

"Prepare a uterotonic. Continue compressions!"

The mother's treatment wasn't over.

It was only beginning.

"Giving it now!"

Once the baby was delivered, the uterus emptied and blood vessels opened.

Bleeding rapidly increased.

They had to hurry.

The uterus had to be sutured.

Then the fascia.

The abdominal muscles.

The skin.

There was no time to waste.

"Oxytocin, ten units IV. Start uterine massage!"

Yuk Jaecheol immediately placed his hand on the uterus and began massaging it to stimulate contraction.

Blood surged, then slowly began to lessen.

"Add methylergometrine, zero-point-two milligrams IM."

Hypothermia reduced the strength of heart contractions.

It also weakened the body's response to medication.

Gain gave another order.

"Blankets. Warm the IV fluids too."

"Body temperature is thirty-five degrees. Applying warm blankets now."

The circulating nurse hurriedly wrapped the patient's legs.

The operating room was noisy.

In one corner, pediatrician Yoon Jihu was working desperately on the baby.

"Baby's heart rate is forty... Continue chest compressions!"

Gain, Jaeheon, and Jaecheol focused completely on the mother.

"Oxygen saturation dropping to sixty-eight."

Kim Jaeheon called out quickly.

"Epinephrine, one milligram IV."

The sound of an ampule breaking was followed by the medication being injected.

"Administered. Starting a three-minute count."

Im Gain gave a short answer.

"Thank you. Continue CPR."

The mother had only about a five-percent chance of surviving without severe neurological damage.

But during the entire ambulance ride, Gain had continuously repeated prayers to the Goddess.

Even though she knew the Goddess wasn't here, she couldn't stop herself.

She had never prayed so desperately before.

Modern medicine plus divine power.

It'll be all right.

A stainless-steel scalpel she could trust.

Reliable disinfectants.

Surgical lights that clearly illuminated the operative field.

All of it made her happier than the handmade mithril scalpel once forged by a Dwarf blacksmith.

But more than anything else...

What she was most grateful for were the other medical professionals around her.

People who shared the same modern medical knowledge.

Especially doctors from specialties outside Gain's own field.

Suddenly, the operating-room door slammed open.

Kang Taehun came running in, breathless beneath his blue surgical cap.

The doctor who had nearly allowed a patient to die because he'd been delayed returning from a conference outside Seoul.

The man who would become a fellow instead of Gain.

"Im Gain? Why are you here? Move."

Taehun tried to push his way beside the operating table.

Kim Jaeheon stopped him.

"Taehun, wait. Switching surgeons right now would be dangerous. The suturing is finished and we're checking hemostasis. I'll keep managing the CPR."

Taehun paused.

Sweat covered his forehead.

His mouth was hidden by the surgical mask, but Gain could easily imagine the twitching expression he always made when irritated.

"...Do you see any more bleeding?"

Without lifting her head, Im Gain answered calmly.

"I got all of it."

Taehun looked as though he wanted to argue.

"You already quit the hos..."

He seemed about to say, You quit the hospital.

But Jaeheon cut in.

"Bleeding's controlled. Oxytocin is still running. Uterine contraction is good. Let's continue CPR."

The hiss, hiss of the Ambu bag suddenly began to sound more regular.

"Oxygen saturation is rising...!"

Kim Jaeheon's low voice came from beside her.

The number on the monitor slowly climbed.

1.

2.

3.

4.

5.

6.

Taehun stared at the monitor without breathing.

"...Gain."

His voice trembled like a whisper.

Im Gain gently pressed her fingertips against the woman's forehead.

"Breathing is returning."

Kim Jaeheon reacted immediately.

"Return of spontaneous circulation confirmed! Heart rate sixty-eight. Blood pressure eighty over fifty."

Taehun slowly exhaled through his mask.

He looked as though he couldn't believe it.

"They said her heart had been stopped for fifteen minutes. She survived...?"

Gain took a moment to steady her breathing.

Then she raised her head.

"She's alive."

For a moment, the operating room fell silent.

The patient had been far more likely to die.

They still didn't know what would happen after survival.

But for now...

They had saved her.

Jaeheon released a long breath of relief.

"Whew."

No one actually stopped working.

But for that brief moment, even the air seemed to listen quietly.

Clang.

Then the outer door opened quickly.

The circulating nurse ran in, breathing hard.

"The guardian is here! Her husband signed the emergency surgery consent form. He's waiting outside!"

Taehun immediately turned his head.

"Really? He's... outside now?"

"Yes. You can prepare to explain everything to him."

Im Gain slowly lifted her gloved hands and looked at the patient's face.

The ECG monitor still showed an unstable rhythm.

But it was unmistakably there.

A beautiful line proving that life remained.

She quietly exhaled.

Now we need to see whether she wakes up...

For a moment, no one in the room spoke.

They simply watched the woman's faint chest movement and caught their own breath after the long silence.

"What about the baby?"

"The baby's heartbeat came back. The baby is breathing too."

Looking at the baby now inside the incubator, pediatrician Yoon Jihu added,

"We're transferring the baby to the NICU immediately."

After confirming the mother's condition, Gain briefly stopped by the NICU before leaving.

She walked toward the incubator and looked down.

The baby's lips, which had been blue before, were slowly becoming pink.

With every small rise and fall of that tiny chest, the monitor showed the heart rate climbing.

Ninety.

One hundred.

Then one hundred ten.

"What was the five-minute Apgar?"

"Four."

"And ten minutes?"

"Six."

The numbers were low.

But the baby had been born without a heartbeat.

And now the child was breathing on their own.

Gain had no way of knowing how much had come from that tiny grain of divine power and how much had come from the baby's own strength.

A tiny hand closed around empty air.

Then slowly relaxed.

Kang Taehun followed Gain.

"I just got here, so I don't know anything. Explain the situation."

His expression clearly said he wanted to avoid responsibility.

Gain had seen that face many times.

She hadn't expected anything else.

"The attending doctor should explain it. I'll summarize the situation for you."

Gain immediately walked out.

Taehun followed behind her.

"The guardian is a judge. Apparently he's a VIP."

The operating-room coordinator whispered.

Taehun stopped walking.

"I'll explain it. Just tell me what I need to say."

"..."

Gain stared at him.

"If it becomes an issue that you performed surgery after submitting your resignation, you'll be in trouble."

Right.

This was what kind of person he was.

He wanted to lick the professors' shoes while looking down on his peers.

"If you leave our hospital, you probably won't have much else lined up. If anyone calls me for a reference check, I'll put in a good word."

"I don't need that. Just listen to the patient's condition."

Gain cut him off.

"Otherwise, how are you supposed to properly explain it after arriving late?"
